What a Complete Male Fertility Evaluation Includes
1. Comprehensive Health & Lifestyle History
We begin with a detailed conversation about your overall and reproductive health. This helps identify factors that may influence fertility, including medical history, medications, past illnesses, sexual and reproductive health, and lifestyle exposures.
Key areas covered:
- Medical history (illnesses, surgeries, medications)
- Sexual and reproductive history
- Heat exposure, environmental and occupational factors
- Lifestyle behaviors that influence fertility
This history lays the foundation for targeted evaluation and care.
You may be asked to bring the following to your appointment:
- previous lab results or medical records
- a list of current medications/supplements
- notes on health, lifestyle, and reproductive history
2. Physical Examination
A focused physical exam can help assess reproductive anatomy and identify factors that may impact fertility. This portion of care is completed by a family physician, urologist, or fertility specialist (MD), and may be recommended as part of a comprehensive male fertility workup.
It may include:
- general health measurements (e.g., blood pressure, BMI)
- exam of the testes and scrotum to assess size, tenderness, or asymmetry
- assessment for varicocele or other findings that can affect sperm health
- additional evaluation as indicated based on symptoms (e.g., urinary changes)
3. Semen Analysis
A semen analysis is a cornerstone of male fertility evaluation. It’s typically recommended at least twice, spaced over 2–3 months, to account for normal biological variation across a full sperm development cycle.
Semen analysis commonly measures:
- Volume: amount of semen
- Concentration: sperm count per mL
- Motility: movement ability
- Morphology: shape and form
Semen testing is most commonly ordered through a family physician, urologist, or fertility clinic, and we can coordinate alongside that pathway. In some cases, Yinstill’s naturopathic doctors can also requisition a semen analysis (an additional requisition fee applies).
Advanced testing may include DNA fragmentation and additional markers (e.g., infection or obstruction) when clinically indicated.
4. Hormonal & Blood Testing
Hormonal balance plays a key role in sperm production, sexual function, and overall reproductive regulation. Bloodwork can help identify contributing factors that may be affecting fertility and guide a more targeted plan.
Testing is most commonly ordered through a family physician, urologist, or fertility clinic, and we can work alongside that pathway. In some cases, Yinstill’s naturopathic doctors can also requisition indicated lab work (an additional requisition fee applies).
Typical assessments may include:
- Testosterone (total & free)
- FSH, LH, prolactin
- Thyroid panel (TSH, Free T3, Free T4)
- Metabolic markers (e.g., glucose; other markers as clinically indicated)
- STI screening where clinically appropriate
5. Genetic Testing (When Indicated)
Genetic evaluation is considered when sperm counts are very low or absent, when there’s a family or developmental history suggesting hereditary factors, or when experiencing recurrent pregnancy loss.
Possible tests include:
- Karyotyping
- Y-chromosome microdeletion panels
- Screening for conditions like Klinefelter’s syndrome
These help identify inherited factors that may affect fertility and guide long-term planning.
6. Advanced Testing When Appropriate
Additional testing is only recommended when it will meaningfully inform care. Examples include:
- Imaging to assess structural factors
- Specialized hormonal or immune assessments
- Repeat or advanced semen profiling
This ensures evaluation is purposeful, not exhaustive.
